Important Note
The following forms:
- D.401, Application for Issue Or Renewal Of Driving Licence (Full),
- D.800, Application for Duplicate Driving Licence (this form can only be used for a licence that has been lost or destroyed and that has not expired),
- D.201, Application for Learner Permit,
cannot be downloaded from the website at this time due to a specific signature requirement which has to be affixed to your licence. They are available at our offices and Garda stations.

- D8(a) Loss of Licence Declaration (Please note : this form is to be used when you are renewing your driving licence and do not have a driving licence to return with your application form. This form is not to be used for replacing a valid driving licence which has been lost or destroyed. To replace a valid driving licence which has been lost of destroyed you must complete a D.800 form.
