Duplicate Licence

If you have lost or destroyed your licence and wish to apply for a Duplicate you are required to produce the following:   Application Form:- Completed application form. Gardai Stamp:- Form must be stamped by the Gardai to report the loss/destruction of the licence. Two identical Passport Photographs:- The reverse of both photographs must be signed by you. Fee:- €10.00 (no fee for applicants over 70 years of age). PPSN - formerly called your RSI number. Note: If you wish to have a duplicate licence issued to you, please bring photographic identification with you. If any details are changed you will need to provide proof of this e.g. utility bill, etc. for address, for change of name, you will need  marriage certificate, passport or birth certificate.
